Mentoring agreement form

This agreement is made between Mentor, Amanda Hill, and Mentee, submitting this form.

During our session we will determine your intentions and get clear about what you are desiring and how to call that in with ease and trust…

I will support you by providing guidance, encouragement and love. 

With this commitment you give your word to:

  • Be coachable

  • Be open to new ideas, concepts and exercises

  • Fulfill on the commitments you make; do the work

  • Powerfully deal with the stressors in your life

  • Begin and end our session on time

MENTORING SESSIONS

Our session will go up to 75mins in duration, unless otherwise agreed. 

TERMS 

This agreement will begin upon acceptance. 

The services to be provided by Amanda Hill to the client are mentoring via Zoom. Mentoring may address specific personal projects, business successes, parenting, or general conditions in the client’s life. Please be on time. 

CONFIDENTIALITY 

Amanda Hill promises the client that all information provided will be kept strictly confidential. The Mentor will keep the Client’s information private, and will not share the Client’s information to any third party unless compelled to by law. 

CANCELLATION 

24hrs notice must be given if you need to cancel a session. If a session is cancelled under 24hrs this session will be missed and not able to be made up, with no refund.

MENTORING RELATIONSHIP 

Throughout the working relationship, the mentor will engage in direct and personal conversation. The client can count on Amanda Hill to be honest and straightforward in asking questions and making requests. 

The client is willing to take responsibility for their life experience and are wanting to create real change in their lives by being honest and doing the work necessary. 

The client understands that the power of the mentoring relationship can be granted only by the client, and the client agrees to do just that: have the mentoring relationship be powerful. If the client believes the mentoring is not working as desired, the client will communicate that belief and take action to return to the power to the mentoring relationship. 

DECLARATION 

I understand that I am working with Amanda Hill for professional mentoring at the agreed fee $555. Since professional mentoring does not constitute medical consultation or treatment, health insurance does not apply. These fees may be considered deductible business expenses.

By continuing this process, you agree to all of the above.
